Morulanın içindeki hücreler ise daha sonra fetusun kendisini oluşturacak olan iç hücre kütlesine dönüşmek için bir araya gelip gruplaşırlar. İşte bu kompleks yapı artık blastosist olarak adlandırılır.
Blastosel sıvıyla doldukça blastosist genişler. Duvarları giderek incelir ve sonuçta kendisini saran zona pellucida’yı yırtarak dışarı çıkar (çatlar) Daha sonra trofektoderm rahimin duvarlarına tutunmaya başlar yani implantasyon gerçekleşir. (ovülasyondan veya tüp bebek tedavisinde yumurta toplamadan sonraki 6-8. günde)
İnsan embriyosu aslında çok ‘müşkülpesent’tir. Yaşayabilmesi için çok spesifik metabolik gereksinimleri vardır. Tüp bebek tedavisi ilk ortaya çıktığında embriyonun içinde gelişmesi için üretilen yapay kültür oldukça ilkeldi ve embriyoların çoğu laboratuvar ortamında en fazla 3 gün yaşayabiliyordu. Dolayısıyla uzun bir süre 3. gün embriyo transferi standart uygulamaydı.
1990’ların ortalarında araştırmacıların geliştirdiği yeni jenerasyon bir yapay kültür sayesinde embriyolar laboratuvar ortamında 5-6. güne kadar gelişebilmeye başladı. Yeni kültür gelişmekte olan embriyonun değişken ihtiyaçlarına cevap verebilecek nitelikteydi. Bu yöntemle günümüzde iyi kalite 3. gün embriyolarının (6-9 hücreli minimal veya sıfır fragmantasyon) yaklaşık %35-40’ı blastosist evresine erişebilmekte.
Burada unutulmaması gereken nokta iyi kalite yumurta üretebilen genç kadınlarda bile embriyoların sadece %35-40’ının blastosist evresine ulaşabiliyor olması. Ancak blastosistlerin (5. gün embriyoların) rahime tutunma ihtimalinin (implantasyon) 2-3. gün iyi kalite embriyolara göre daha yüksek olması nedeniyle , iyi kalite blastosist transferi daha yüksek tüp bebek başarısı sağlamaktadır.
Araştırmalar göstermektedir ki blastosist evresine ulaşabilen embriyoların büyük çoğunluğu kromozomal olarak normal (euploid) embriyolardan gelişmişlerdir ve bu nedenle normal bebeklere dönüşme ihtimalleri daha yüksektir. Blastosist evresine ulaşamayanların çoğu da kromozomal olarak anormal embriyolardan gelişmişlerdir.
Blastosist evresine ulaşan embriyoların CGH adı verilen yöntemle en kalitelilerinin seçilmesi tüp bebek tedavisinin başarısını daha da artıran bir gelişmedir. Bu sayede transfer edilen embriyo başına hamilelik oranı artmış, düşük ihtimali ve kromozomal doğum anomalileri azalmıştır.
Blastosist transferi (5. gün transferi) tüp bebek başarısını artırmakla birlikte tüm hastalarda uygulanabilen bir yöntem değildir. Eğer hastanın çok sayıda (>4) iyi kalite embriyosu varsa 5. güne yani blastosist evresine kadar beklenerek hangi embriyoların daha yüksek kalite olduğu gözlemlenebilir. Ancak 3. günde 6-8 hücreli embriyo sayısı 4’ün altında olan ve/veya daha önce tüp bebek başarısızlığı yaşamış hastalarda daha fazla beklenmeyerek 3. gün transferi tercih edilir. Böylece embriyolar ‘doğal’ ortamlarına daha erken yerleştirilmiş olurlar. Kalan embriyolar dondurulacaksa 3. gün transferi yapıldığında daha fazla sayıda embriyo dondurma şansı vardır.
Özetlemek gerekirse:
1. Tüp bebek tedavisinde döllenmeden sonra 5-6 gün beklenerek blastosist evresine ulaşabilen embriyoların transfer edilmesi pek çok kalitesiz ve anormal embriyonun elenmesini sağlar. Böylece implantasyon ihtimali yükselir ve tüp bebek başarısı artar. Bu evrede embriyolara uygulanabilen CGH yöntemiyle en kaliteli blastosistlerin seçilmesi ise bu başarıyı daha da artırır.
2. Ancak bu evreye kaliteli 3. gün embriyolarının sadece %35-40’ı ulaşabildiğinden transfer edilebilecek ve istenirse dondurulabilecek embriyo sayısı oldukça azalmış olur.
3. Bu nedenle döllenmeden sonra 3. günde 6-8 hücreli embriyo sayısı 4’ün altında olan ya da daha önce başarısız tüp bebek denemeleri olan hastalarda 3. gün embriyo transferi önerilir.
